Just recently upgraded from a ps4 to a ps5 and lost access to the forsaken pack, shadowkeep, and the 30th anniversary edition. I bought the deluxe witch queen bundle which came with came with the 30 anniversary pack and I owned the forsaken and shadowkeep expansion since the initial release. I have receipts to prove this if needed. I tried restoring licenses on the ps5 but nothing changed.
